Jeffrey H. Garber, O.D.

Optometrist Dr Jeffrey H Garber Winston SalemDr. Garber provides highly skilled primary comprehensive eye care, having managed and served the eye care needs of Kernersville and the triad community for over 25 years. As founder of Garber Eye Center, he has provided eye exams, contact lens diagnostic evaluations, treatment of eye diseases, emergency eye care, ophthalmic procedures, surgical consultative care and surgical co-management to over 250,000 patients.

Dr. Garber's educational degrees include a Doctor of Optometry, Bachelor of Science in Biology, and Bachelor of Arts in Psychology from North Carolina State University and Southern College of Optometry along with an associate degree in Opticianry. Dr. Garber is a member in good standing of the North Carolina State Optometric Society, Inc., the American Optometric Association, and the local Winston-Salem Optometric Society.

Dr. Garber has lived in Kernersville over 26 years and has surrounded himself with the best doctors and staff, all dedicated to providing premium eye care to the Kernersville community.

 

Brian P. Harris, O.D.

Optometrist Dr Brian P Harris Winston SalemDr. Harris received his undergraduate degree in Biology from the University of Illinois. He then attended the Illinois College of Optometry in Chicago where he graduated Doctor of Optometry in 1994. He also received training at the Veterans Administration Medical Center in Huntington, WV. Dr. Harris graduated a member of the Beta Sigma Kappa Optometric Honor Society and the Tomb and Key Optometric Honor Fraternity. Upon graduation, Dr. Harris worked for 5 years in multiple practice settings, including ophthalmology.

Dr. Harris joined the staff at Garber Eye Center in 1999. He provides eye care including contact lenses, management of diseases of the eye and emergency eye care. Dr. Harris is a member of the Triad Optometric Association.

Dr. Harris resides in Greensboro with his wife and four children. He enjoys tennis, running and playing guitar.

 

Benjamin D. Crowell, O.D.

Crowell, O.D.Dr. Crowell received his Bachelor of Science degree in Biology with a minor in Chemistry from the University of South Alabama.  While completing his undergraduate degree, he worked several years as a certified optician and store manager for Luxottica. He then attended Nova Southeastern College of Optometry in Fort Lauderdale where he graduated with academic and professional honors receiving his Doctorate of Optometry and a Sigma Kappa Optometric Honor Society and was honored with a primary care clinical achievement award.  Dr. Crowell continued his clinical training through an externship at Bascom Palmer Eye Institute in Miami, FL.  After graduation, he went on to complete additional training through a residency specializing in the treatment of ocular disease and primary care at the W.F. “Bill” Hefner VA Medical Center in Salisbury, NC.

Dr. Crowell has given educational lectures at local and national meetings and has been published in optometric literature.  His active memberships include the American Optometric Association, the North Carolina Optometric Society, and the American Academy of Optometry where he is a candidate for Fellowship.  Clinical interests include glaucoma, contact lenses, diabetic retinopathy, dry eye, emergency eye care, and cataract/LASIK co-management.  He looks forward to meeting and serving you and your family with your eye care needs.

Neill J. Bulakowski, O.D., F.A.A.O.

Optometrist Winston Salem | Dr. Neill J. Bulakowski, O.D. Dr. Bulakowski, attended Indiana University in Bloomington, IN, where he received a Bachelor of Science in Biology in 2003 and a Doctor of Optometry in 2007.  Upon graduation, Dr. Bulakowski completed a year of residency training in hospital-based optometry, focusing on primary eye care and ocular disease, at the Perry Point Veterans Affairs Medical Center in Maryland.  In 2009, he completed his fellowship in the American Academy of Optometry.

Dr. Bulakowski graduated a member of Beta Sigma Kappa International Optometric Honor Society.  He remains active in the American Academy of Optometry, American Optometric Association, and the North Carolina State and Local Optometric Chapters.

New to the area, Dr. Bulakowski is excited for the opportunity to become active in, and provide eye care to, the Triad Community.  He and his wife reside in Greensboro where they enjoy traveling, music, and the outdoors.

 

Terry W. Ellington, O.D., F.A.A.O.

Optometrist Dr Terry W Ellington Winston SalemDr. Terry Ellington has lived almost his entire life in the triad area. He attended Eastern Randolph High School in Ramseur where he played football, wrestled and was on the track team. College was at the University of North Carolina at Greensboro where he earned a biology degree graduating magnum cum laude. This education allowed Dr. Ellington to pursue the career of an eye doctor at the Southern College of Optometry in Memphis, Tennessee. While in Memphis he was awarded a Letter of Commendation from the US Navy's Branch Medical Clinic for service during the immediate time after 9/11. In 2000 he earned a second bachelor's degree in Physiological Optics and in 2002 he received his Doctor of Optometry degree.

Prior to joining Garber Eye Center, Dr. Ellington practiced in Winston-Salem and High Point. Besides North Carolina, he has held licenses to practice optometry in Pennsylvania, Virginia and South Carolina. He is a member of the American Academy of Optometry, the American Optometric Association and the local Winston-Salem district of the American Optometric Association.

Dr. Ellington and his wife Wendy live with their three sons, Xander, Ethan and Carter, in Davidson County. Dr. Ellington enjoys spending his free time with not only his wife and boys, but his family still living in Randolph County.
